INVITATION TO BID (ITB) - Transportation
Hardin County Department of Job and Family Services is issuing an lnvitation to Bid (ITB to solicit quotes from qualified providers for transportation services for Medicaid eligible individuals as well as any other AGENCY approved transportation. Services to begin July 1, 2019 through June 30,2021 with a mutual agreement that we could extend it another year for a total of 3 years with no increase in rate or change in services provided. Extension would be contingent upon availability of funds, including compliance with all applicable budgetary and legal reguirements and satisfactory performance by the contractor. Bid must provide cost of transportation one way, idle time and proof of appropriate insurance for liability, loss, damage, and/or related expenses incurred through provisions of services. Alt bids will be evaluated, and selection based upon the criteria outlined in the ITB. HCDJFS will reserve the right to accept or reject any and all bids, the right to waive any informalities in the bids received, and to cancel in part or in its entirety the ITB. REQUEST FOR Quotation - Contracted Services
Hardin County Department of Job and Family Services is accepting quotations for the contracted services listed below, with the effective date July 1, 2019 through June 3Q 2021 with a mutual agreement that we could extend it another year for a total of 3 years with no increase in rate or change in services provided. Service: ln- Home Service Provider/ Family Coach
a. Works with AGENCY Case Manager to develop an intervention strategy which is designed to assist the family in overcoming a specific barrier or problem. lnforms Case Manager whether or not intervention strategy being requested is within the scope of the PROVIDERS background and ability to provide such services.

b. Provides documentation of family progress or lack of progress on AGENCY provided forms.

c. Maintains skict confidentiality standards as outlined in AGENCY Confidentiality Agreement, attached herein and incorporated by reference.

d. Prepare and testify in court when requested. e. Transportation of clients as needed to appointments- Proof of valid driver/s license and automobile insurance must be provided.

f . Other services as mutually agreed upon will be included in the contract.
